Getting a UDID card: a step-by-step guide

The UDID (Unique Disability ID) is a single card that records your disability details and works as proof across government schemes. Here is how to apply online.

What the UDID card is

The Unique Disability ID (UDID) project creates one national database and one card for every person with a disability in India. Instead of carrying several documents, you get a single card that many schemes accept as proof — including ADIP and benefits under the National Trust.

Before you start

Keep these ready as scans or clear photos:

  • A passport-size photograph.
  • Identity proof (Aadhaar, voter ID or similar).
  • An existing disability certificate, if you already have one.

If you do not have a disability certificate yet, the same portal lets you apply for one; a government medical authority will assess you and issue it.

How to apply

  • Go to the official Swavlamban Card portal and register with your name, mobile number and email.
  • Fill in your personal, disability and employment details.
  • Upload your photo and documents.
  • Submit the form. Your application goes to the Chief Medical Officer (CMO) or medical authority in your district.
  • After assessment and approval, your UDID card is generated. You can download an e-copy and the physical card is sent to you.
